Job Description:
We are seeking a highly skilled and dedicated Clinic Nurse Practitioner to join our team at ChenMed. As a Clinic Nurse Practitioner, you will be responsible for providing direct patient care in a variety of settings, including medical centers, nursing homes, skilled nursing facilities, and home settings.
Responsibilities include:
- Conducting geriatric assessments and medical histories
- Performing physical exams and making diagnoses
- Developing and implementing nursing plans of care
- Providing health education and counseling to patients and families
- Referring patients to specialists as needed
- Participating in risk and quality management programs and clinical meetings
The ideal candidate will have a strong background in nursing and experience working with geriatric patients. A master's degree in nursing is required, as well as current certification as a nurse practitioner.
