Job Description
The Primary Care Physician (PCP) plays a vital role in our clinical operations team, providing direct patient care and assessments primarily in the ambulatory healthcare setting. This position involves:
- Geriatric assessment
- Medical history and physical examination
- Diagnosis and treatment
- Development of the plan of care
- Health education
- Specialty referrals and case management
- Follow-up and clear documentation according to ChenMed standards
The PCP will work independently and collaborate with other healthcare professionals, consulting with applicable managers and medical directors to ensure compliance with guidelines. They will participate in risk and quality management programs, clinical meetings, and other required meetings.
Key Responsibilities
- Functions independently as a primary care practitioner as part of a patient care team.
- Assesses acute and non-acute clinical problems.
- Performs and documents physical assessments and patient histories.
- Develops, documents, and implements a patient management plan.
- Provides patient and family education.
- Serves as a patient advocate.
The PCP will adhere to strict departmental goals, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
Salary : $200,000 - $250,000
