ABOUT THE JOB: Summary:
The Health Coordinator facilitates patient continuity of care with the healthcare team. This position is responsible for utilizing available resources to ensure patients/families receive care that is continuous, comprehensive, and coordinated across the care continuum. The Health Coordinator works cooperatively with providers and personnel to carry out the goals and objectives of ambulatory care management according to established policies and procedures.

- At least 4 years' experience at Pediatric/Family Practice or Pediatric Med/Surg floor preferred
- High school diploma or equivalent required
- MA, CNA or other healthcare related certification required
