Co-Occurring Case Manager

Purpose of this Position:

Co-Occurring Case Managers provide assistance to persons (ADULTS) with serious and persistent Mental Illness and a Substance Use Disorder. The case manager helps individuals gain access to needed resources such as medical, social, educational, natural supports, community resources and specialized mental health treatment, substance use disorder treatment, rehabilitation and support services. This service is expected to achieve specific outcomes of independent living, vocational/educational participation, adequate social supports and reduced hospitalizations.

Responsibilities Include, but are not limited to:

• Knowledge of mental health, substance use disorders and social service field 
• Familiarity with and ability to access community resources 
• Ability to make accurate assessments of clients’ needs and to be creative in seeking appropriate services 
• Ability to organize time efficiently to meet client needs in a flexible, non-traditional time frame

Qualifications:

• Bachelor’s degree in Social Work, Psychology, Sociology or related social science field
  • Two (2) years’ experience (or equivalent combination of experience and training) in a public or private human services organization
