What are the responsibilities and job description for the Managed Care Options Counselor position at CICOA?
This position is responsible for assessing the needs of individuals and/or their caregivers when they are referred by provider sources. In addition, this position, provides information and referral services enabling them to make informed decisions based on available options.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Work independently to meet CICOA HCC department goals and objectives at a high rate of success as stated in the scope of work.
- Demonstrate leadership qualities to improve HCC programming, co-develop goals and objectives, identify ways to improve service delivery, increase efficiency, and improve team moral.
- Respond quickly to referrals according to CICOA and contracted agency guidelines as detailed in the scope of work.
- Respond to client’s usage of the health system by reviewing information entered by health systems throughout Indiana to improve efficiency, lower health care costs, or improve quality of care.
- Develop effective/professional working relationships with the contractor’s staff, health system’s interdisciplinary team, and CICOA staff.
- Work as a member of multiple teams across a variety of settings.
- Develop recommendations for care plan development for assigned clients including formal and informal supports.
- Participate in case conferencing and rounds with the contractor’s staff, health system, and CICOA staff.
- Problem solve in the field by identifying appropriate community resources for client’s, caregivers, health care and other social service professionals.
- Conduct assessments in a hospital setting and/or at the client’s residence to determine needs and provide resources as appropriate.
- Maintain flexibility in scheduling assessments to meet client and/or caregiver schedules.
- Display competence for guidelines, rules/instructions set by the contractor, Medicaid Waiver, CHOICE, SSBG, Title III, and Pre-Admission Screening.
- Maintain daily, weekly, and monthly activities in multiple database tracking systems.
- Provide ongoing care management services assigned by CICOA or contractor.
- Record time and tasks according to agency and funding source guidelines.
- Assist in training new Health Care Collaboration staff.
- Develop and maintain working relationships with area organizations and service providers.
- Demonstrate agency’s customer service standards.
- Conduct outreach activities for service referrals.
- Attend scheduled trainings/meetings.
- Participate in advocacy activities as assigned.
- Perform other duties as assigned.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE
High school diploma or equivalent required; degree in human services, psychology, nursing, or related field preferred; computer experience (Microsoft Office Suite, internet, email) required. Two years of full-time, direct service experience working with individuals with complex medical or social needs which includes assessment, care plan development, monitoring, and engaging members preferred. Health services/relatable experiences working with older adults highly preferred.
CERTIFICATES, LICENSES, REGISTRATION, OTHER
Community Health Worker (CHW) certification or the ability to complete training and achieve certification within first six months of hire. Must possess a valid Indiana Driver's License and provide evidence of auto liability insurance. Complete certification or training required to perform contracted duties.
