Position Summary:
The Clinical Service Supervisor (RN) plays a role in providing direct skilled nursing services and clinical oversight for care delivered across home and community settings. This role ensures that services meet person-centered standards and comply with regulatory guidelines
Key Responsibilities:
Clinical Oversight & Medical Supervision
- Conduct initial and periodic nursing assessments and participate in plan-of-care development.
- Maintain a consistent caseload with regular oversight visits to individuals in Home and Community settings.
- Supervise and evaluate the performance of LPNs, CNAs, PCAs, and Companion caregivers.
- Provide ongoing clinical monitoring to ensure accuracy in medication administration, documentation, and overall care.
- Review and ensure accuracy of MARs, ISPs, HCPs, RMPs, HRST assessments, and incident reports.
- Conduct supervisory visits to assess caregiver performance, ensure client satisfaction, and provide guidance on care practices.
- Develop and implement Risk, Obstacles, and Barriers (ROB)-based Care Plans.
- Participate in incident investigations, mortality reviews, and corrective action planning (CAP).
- Educate staff on regulatory updates, department guidelines, and health and safety protocols.
Direct Nursing Care & Support
- Deliver skilled nursing services and treatments as prescribed.
- Monitor and manage medications and treatment regimens.
- Provide health education to clients, families, and caregivers regarding medical conditions and care needs.
Training & Staff Development
- Conduct onboarding and subsequent training for new caregivers and nurses.
- Perform direct observation and competency evaluations of supervised staff.
- Provide in-service training on medication protocols, infection control, and emergency response procedures.
Compliance & Documentation
- Maintain accurate, timely, and compliant documentation in agency systems and related portals.
- Ensure clinical operations align with department licensure standards, Waiver Manuals, and compliance requirements.
- Prepare documentation for audits, QA reviews, and regulatory reporting.
Communication & Coordination
- Act as liaison between clients, families, caregivers, support coordinators, and healthcare providers.
- Collaborate with interdisciplinary teams to ensure continuity and coordination of care.
- Provide timely responses to questions or concerns related to health plans, medications, and service delivery.
Required Qualifications:
- Current and unencumbered RN license in Georgia
- Associate’s Degree in Nursing required; BSN preferred
- HRST Clinical Reviewer Certification (or willingness to obtain)
- Valid Georgia driver’s license and reliable transportation
- Current CPR certification
- Negative TB test and satisfactory background check
Experience:
- Minimum 3 years of nursing experience, experience with DD population and home care settings
- Experience in waiver programs and CMS regulatory environments, preferred
- Familiarity with licensure rules and department standards for health services
- Prior experience supervising LPNs, CNAs, and direct care workers preferred

Work Environment & Physical Requirements:
- Work is primarily performed in client homes and occasionally in the office
- Must be able to travel frequently within assigned service areas
- Physical ability to assist with client mobility and perform nursing duties
- Flexible scheduling, including some evenings and weekends, may be required
