As our agency DDP, you will be responsible for overseeing the implementation of care plans and services for adults with intellectual and developmental disabilities (IDD). The DDP ensures that individuals receive high-quality, person-centered support that meets all DBHDD and waiver requirements. You will play a vital role in ensuring the effective delivery and management of quality client care and other essential office duties. This position reports directly to the Assistant Administrator and plays a vital role in ensuring compliance, safety, and continuity of care across the agency’s service network.

Key Responsibilities
- Maintain a case load of up to 40 clients, ensuring timely and consistent support
- Conduct in-person visits with each client every 60-90 days. Ensures that clients and families are satisfied with CCHS services, works to find solutions to issues.
- Supervise and support individuals with IDD in accordance with their Individual Service Plans (ISP)
- Attend ISP meetings with Support Coordinators and family.
- Monitor and evaluate service delivery, ensuring compliance with ISP goals and objectives
- Participate in interviewing, recruitment, and orientation of new direct care staff.
- Train and support direct care staff/DSPs to ensure they are meeting the needs of each individual appropriately
- Conduct and participate in staff training, ISP meetings, and assessments (SIS, HRST)
- Develop and oversee crisis plans and high-intensity service strategies when necessary
- Ensure compliance with Medicaid and DBHDD policies, provider policies & procedures, and protocols and documentation meets DBHDD standards and agency policies
- Participate in the development and revision of ISPs and implementation strategies
- Serve as a liaison between families, staff, and support team members
- Maintain ongoing communication with clinical and administrative staff
- Work closely with senior management to improve office workflow and efficiency.
Minimum Requirements
- Bachelor’s degree from an accredited college or university in a human services-related field (as defined by DBHDD standards)
- CPR/First Aid certified (or willing to obtain)
- Negative TB Test
- Clean driving record and criminal background check
- Excellent written and verbal communication skills
- Strong interpersonal and organizational skills.
- Must be at least 21 years of age
- Minimum 16 hours of yearly training
Preferred Requirements
- Prior experience in home community care or in a related healthcare role is preferred.
- Proven ability to assess and manage clients in a home care setting is preferred
- Able to manage a homecare office and daily operations.
- Some experience in recruiting employees
- Prior Georgia Medicaid Waiver (NOW & COMP, EDWP, etc) experience
- Prior case management experience
- Experience working with adults with intellectual and developmental disabilities (IDD)

Job Types: Full-time, Part-time
Pay: $20.00 - $22.00 per hour
Expected hours: 16 – 40 per week
