What are the responsibilities and job description for the Public Safety and Justice Case Coordinator position at City of Columbia, MO?
The purpose of this job is to serve as a case manager in the Divert Program to address health and social issues in an effort to reduce their burden on public safety and justice systems and to improve system level performance and individual health and well-being. Conducts case management and crisis intervention for high-risk individuals.
Essential Duties and Responsibilities - The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by employees in this job. - Provides intensive case management for individuals identified as being at risk due to complex social and health issues.
- Provides crisis intervention services.
- Provides home visiting and street outreach services in collaboration with public safety representatives.
- Provides health information and training.
- Collaborates with internal and external partners to facilitate the Divert program.
- Performs data management and analysis
- Prepares and presents reports.
- Assists in internal process evaluation and improvement. Applies performance management and quality improvement methods and tools to improve organizational performance.
- Regular attendance is a necessary and essential function.
- Performs other duties as assigned.
Supervisory Responsibilities
- No supervisory authority. Work may require providing guidance and training to others.

Education and Experience - An equivalent combination of education, training and experience will be considered
Education and Experience
- Bachelor’s Degree in Social Work, Family & Human Development, Social Sciences or related field.
- 3 to 5 years’ experience in providing intensive case management to diverse populations.
Licenses or Certifications
- Valid driver’s license.
- Registered with the Missouri Family Care Safe Registry within 60 days of hire.
- Homeless Management Information System Certification within 60 days of hire.
