Supportive Housing Coordinator- Home Based Services
Program: Home Based Services
Location: Arlington Heights, IL
Hours: Full time, Monday - Friday, 40hrs/week
Clearbrook is a non-profit, social services agency that specializes in providing innovative services to individuals with intellectual and developmental disabilities, such as Autism and Down Syndrome.
The Supportive Housing Coordinator is responsible for managing all activities related to the Supportive Housing. The Supportive Housing Coordinator reports to the Director of Home-Based Services.
The SHC must have:
- Bachelor's degree or higher in human services, social services, or health related field
- At least one year of experience working directly with individuals with intellectual and developmental disabilities, such as Autism and Down Syndrome
- 2 years supervisory experience
- Valid IL driver's license
Essential Functions:
- Demonstrate a thorough knowledge and understanding of the Department of Human Services regulations and ensure compliance with these guidelines and regulations.
- Keep apprised of current practices and developments in service provision and apply these principles to the Home-Based Program when applicable.
- Attend staff meetings for the effective operation of the program and the proper and timely dissemination of information.
- Conduct and attend trainings in accordance with the training policy of the Home-Based Program.

- Provide direct service facilitation in a Self-Direction Assistant capacity to a limited number of individuals in the Home-Based Support Services Program, which includes home visits, service planning, assisting and managing monthly budgets as appropriate, and maintaining complete records.
- Provide supportive housing resources to Home-Based Services and community participants.
- Keep apprised of current supportive housing resources and contacts.
- Conduct / facilitate family groups looking for innovative, supportive housing through training, collaboration and resource referral.
- Develop and implement sound financial management of the program in cooperation with the Director of Home-Based Services.
