Join our Esplanade House Transitional Housing Program as a Lead Care Manager, where your work will directly contribute to transforming the lives of families as they journey through transitional housing and toward self-sufficiency.

As a Lead Care Manager, you will be at the forefront of helping vulnerable families break the cycle of homelessness. You’ll use your expertise to offer tailored support, advocate for their needs, and connect them to critical resources. Your role will also include helping families navigate the complex housing system, securing permanent housing solutions, and connecting them with essential support services that will help them build a stable future.

Key Responsibilities:

  • Be a Beacon of Support: Engage with Medi-Cal eligible families, offering hands-on support where they live and seek care.
  • Guide Families to Success: Oversee and deliver Enhanced Care Management (ECM) services by completing assessments, developing personalized care plans, and supporting their journey to wellness.
  • Housing Navigation: Assist families in navigating the housing system, helping them secure permanent, safe, and affordable housing. This includes identifying housing opportunities, advocating with landlords, and connecting families to housing resources.
  • Advocate and Collaborate: Work with healthcare providers, hospital staff, housing authorities, and other community organizations to ensure comprehensive support and avoid service duplication.
  • Innovate and Empower: Use motivational interviewing, trauma-informed care, and harm-reduction techniques to support families in a compassionate, understanding way.
  • Be a Resource Connector: Link families with transportation, social services, and community supports, empowering them with the tools they need to succeed.

Your Impact:

Your primary focus will be case management, working closely with families to create personalized, actionable plans that cater to their health, social, and housing needs.

What We’re Looking For:

  • Passion: A deep desire to make a difference in the lives of families facing housing instability.
  • Experience: At least two years of case management experience in a related area (or equivalent experience), with a strong focus on housing navigation and advocacy.
  • Leadership: The ability to guide, support, and empower team members and program participants.
  • Communication Skills: Strong verbal and written skills to effectively advocate for families and collaborate with various community partners.
  • Bilingual Skills: A plus! We offer incentives for bilingual candidates.
