Job Description:
Help us improve patient care and employee and patient satisfaction! Home & Health Care Management is looking for a full-time self-motivated, reliable, computer savvy RN to join our team. Will be managing our Intermittent, Private Duty, HIV/Aids waiver programs. This is a full-time 40-hour-a-week position in our Chico Office. The hours are Monday- Friday 8:00 am – 5:00 pm with an hour lunch. This is a full-time exempt position in Chico, CA with occasional travel to clients’ homes in surrounding areas.
Responsibilities:
· Responsible for monitoring of agency quality assurance/improvement programs; orient new clinical staff to home health care documentation, electronic health records and interdisciplinary, patient centered care planning processes.
· Provide ongoing continuing education opportunities for existing staff as necessary.
· Respond to medical record review requests from Medicare, MediCal and or private insurance payers.
· Review or develop and maintain clinical policy and procedures for the agency.
· As part of the management team, interface with human resources, accounting/billing as well as clinical staff and patients and their caregivers.
· Must be able to attend and manage monthly case conference meetings.
· Must be able to make home visits when needed.
Experience & Education & Other Requirements:
· Must possess a California RN license.
· Must have worked as an RN for at least 2 years.
· Must have working knowledge of Microsoft office products and computer literate.
· Must have current knowledge of current regulatory and clinical practice standards
· Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations
· Must effectively plan, organize staff and resources, and prioritize work, thinking critically to effectively organize operations consistent with HHCM strategy and effectively solving problems as they arise
· Must have oral and written communication skills
· Must have interpersonal skills and role model customer service skills
· Must establish and maintain positive and effective working relationships with staff, physicians, patients, and other departments
· Must be able to work independently and manage competing priorities while maintaining a productivity standard set by the Transitional Care Director.
· Must be able to pass a pre-employment background check including fingerprinting.
· Must have 5 years of related experience with at least 3 years of management experience required.
· Must have reliable transportation and ability to drive in all terrains and weather conditions throughout agency service area. Clear driving record from DMV.
· Must be able to work independently and manage competing priorities while maintaining a productivity standard set by the Transitional Care Director.
· Must be able to pass a pre-employment background check including fingerprinting.
· Must have the ability to be a consistent mentor to staff and use the disciplinary process appropriately
· Must have a valid California driver’s license and insurance and ability to drive oneself on all roads & weather conditions.
· Must provide DMV driving record.
· Agency requires all employees to show proof of full COVID vaccination or have an exemption. PPE is provided to all employees
· Required to work Monday through Friday, 8:00 am to 5:00 pm with an hour lunch
Preferred experience & traits:
· Prefer home health care/ hospice and/or community-based settings.
· Persons with a Bachelor’s, Master’s Degree in Nursing or related degree are encouraged to apply, however experience in the field can substitute for education.
· Computer skills, including competency with MS Office suite (Outlook, Word, Excel)
· Efficiently use electronic medical records systems.
· Flexibility and independence required to work competently and autonomously
· Self-started with a high degree of initiative, motivation, flexibility, and creativity
· Knowledge of Oasis, Title 22 and CMS regulations
