What are the responsibilities and job description for the Case Manager position at Community Care Alliance?
This grant-funded specialist case manager works closely with an experienced RN to enhance Integrated Health Home care for medically complex clients with mental illness and/or substance use disorders.
Responsibilities include assessing clients’ safety and functioning at home, assisting with specialty medical care coordination and appointment attendance, communication with clients’ natural supports and IHH team members, and other duties as assigned.
Expectations include reliable, on-time work attendance, adherence to HIPAA requirements, timely and thorough documentation in an electronic health record, maintenance of registered and insured vehicle, and ability to function cooperatively as part of a multi-disciplinary team. 20-40 hours Monday to Friday, depending on availability of ideal candidate.
