A Case Management Supervisor manages an extensive caseload of individuals with disabilities in community-based jobs in Philadelphia, Delaware, Bucks, Montgomery and Chester Counties. They independently create a weekly schedule to provide job supports when needed to help the participant become as independent as possible. The Case Management Supervisor oversees and maintains funding compliance with all funders, and they serve as the liaison between the employer, family, participant and other essential team members.
Brief Summary of Job Duties:

Oversee an extensive caseload, approximately 30-40 participants in Philadelphia, Delaware. Bucks, and Montgomery and Chester Counties.

Independently schedule and provide job support to participants on site as needed in their employment settings.

Serve as the liaison between the employer, family, participant and other team members.

Track own billable data and submit daily in Setworks.

Review and update TL/FP’s and other documents when needed.

Monitor and initiate fading.

Attend meetings as needed.

Create ISP Videos.

Update information in Setworks as needed and organize SharePoint folders as needed.

Cover coaching shifts, meetings, and interviews when needed.
Qualifications

Associate or Bachelor's Degree preferred (four year college or technical school)

1 plus years of experience in Education, Social Services or Business preferred.

Ability to effectively use Microsoft Office products, specifically Outlook, Excel and Word.

Possess valid driver's license and automobile insurance.

Must have access to an insured vehicle.

Must have a home computer with internet access and video capabilities.

Able to pass all required background checks.

Must be fully vaccinated or in the process of getting fully vaccinated.
