Position Summary:
The Perinatal Care Coordination Specialist is responsible for providing supportive services and education to perinatal families with an emphasis on getting ready for birth and baby, addressing barriers and gaps in care in order to improve birth outcomes among women who are at higher risk for adverse birth outcomes.
In your day to day role, you will be responsible for conducting intakes, assessing for high-risk social needs and following up appropriately, identifying additional possible parental stressors, utilizing healthy screening tools, providing health and reproductive education, coordinating appropriate referrals and linkages to other support services, timely data entry, and fostering relationships with the community and other social agencies and programs through outreach activities. This is a full–time position.
Highlighted Duties and Responsibilities:
  • Provides care coordination services to ensure access to perinatal health and early child health services;
  • Provides telephone and/or face-to-face care coordination services with families on caseload, at regularly scheduled intervals, depending on risk;
  • Conducts thorough assessments on all pre/postpartum patients utilizing OB Authorization form, Healthy Screening tools (HITS, 4P’s, PHQ-9, Edinburgh); Refers and follows up with patients as appropriate, based on risk;
  • Assists as a liaison between the patient and their families, the provider, clinical staff, and other health care professionals;
  • Develops care plans with designated periods with emphasis on risk factors, including but not limited to social needs, barriers to care, pregnancy planning, and food insecurity;
  • Conducts outreach, educating the community on COH prenatal services and promoting COH program;
  • Provides referrals to community resources when necessary, including housing, home visiting, WIC, dental, breastfeeding, and job training program;
  • Enters data accurately and timely into medical records (eCW) or other databases regarding care plans, referrals, assessments, and completion of services and referrals, mental health supports, etc.
Minimum Qualifications:
  • A.A. or B.A I n Human Services field (social work, early childhood education, family studies, or psychology) preferred or equivalent professional experience required.
  • At least 2 years experience in public health and/or nonprofit sector, working with underserved populations;
  • Experience with providing health education and promotion to ‘at risk populations. Reproductive health background preferred;
  • Familiarity with Medical Assistance system and community resources;
  • Demonstrated cultural competence with communicating with low income, culturally diverse populations;
  • Strong written and verbal communication skills;
  • Ability to work with computers and electronic health records.
