Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 10 states.
We are seeking a Medical Coordinator in Philadelphia, PA! The Medical Coordinator will plan, track, monitor and ensure that all clients’ needs are being met within appropriate time frames. Primary responsibilities include; the scheduling of all annual, follow-up and emergency medical appointments. These include communicating with medical professional staff, program specialists, supervisors and program directors.
Responsibilities:
- Under the direction of the Program Specialist, primary responsibility is to schedule all client medical appointments to state mandated licensing regulations
- Observes and monitors the client’s status through paperwork and follow-up appointments.
- Observes and tracks clients' physical care including, but not limited to annual and emergency appointments.
- Provides accurate, timely, and consistent documentation of client medical appointments in the appropriate records
- Attend all pertinent medical appointments that need to be addressed more clearly and precise to achieve the best results for the individual
- Communicates with supervisors, co-workers and medical professionals’ regarding clients’ status
- Cooperate with audits, inspections, and investigations
- Additional tasks and responsibilities may be assigned
Minimum Requirements:
- High School Diploma or GED required; Bachelor’s Degree preferred
- Valid driver’s license with a satisfactory driving record
- Complete all state and agency required training per state guidelines
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
- Excellent verbal and written communication skills
Working Conditions:
- May be required to be on-call in cooperation with other management staff
- May be required to transport individuals utilizing your own vehicle or company provided vehicles
