What are we looking for in a Care Coordinator?
The Care Coordinator provides outreach, case management, and supportive services to individuals who have serious mental illness. This position provides support to two programs in Yakima: our walk-in clinic for primary care, which is open to the general public with emphasis on supporting our existing behavioral healthcare clients, and our collaborative behavioral health project with Camp Hope, a low-barrier emergency homeless shelter for adults and families. The Care Coordinator works to engage clients in treatment and reduce barriers to care. Meeting weekly with each client, the Care Coordinator works to determine their needs and goals, helps to educate about available community resources, and supports clients in developing the confidence and skills necessary for self-advocacy, independent living, and social participation. The Care Coordinator also serves as a liaison between the client and their treatment team to advocate for the client’s needs and collaborate with other community partners to coordinate services. The Care Coordinator supports clients by developing trusting relationships, overcoming resistance to the use of behavioral health services, and empowering clients to be the driving voice in their treatment. This role requires strong communication skills, the ability to build rapport quickly, and sincere compassion for clients who are experiencing significant individual and systemic challenges. This role has a schedule of Monday-Friday 8am-5pm.
Care Coordinator duties may include:
- developing and maintaining trusting relationships with clients by demonstrating patience, open-mindedness, nonjudgment, compassion, flexibility, responsibility, and follow-through
- collaborating with clients to identify needs and challenges and to develop an individual care plan that increases client activation, access, and appropriate use of services and supports crisis diversion
- evaluating and modifying support as necessary based on client progress and input from the client and treatment teams
- utilizing motivational coaching to assist clients with developing skills to access, enroll in, and utilize resources and services related to transportation, housing, food, and other basic needs
- assisting clients with making and getting to appointments and helping clients to overcome their resistance to utilizing services
- mentoring and supporting clients as they work though their treatment plans to reinforce successes and address setbacks
- monitoring clients’ symptoms, treatment progress, and medication response
- completing all documentation in a respectful, client-centered manner
- demonstrating knowledge of CCBHC model of care for integrated behavioral health systems
Qualifications:
- Bachelor's degree in social work, psychology, or related field
- Knowledge of social determinants of health and related challenges, as gained through lived experiences
- Prior experience with individuals experiencing homelessness is preferred
- Must complete Community Health Worker training within six months of hire
- Must be eligible for registration as Agency Affiliated Counselor
- Must have driver's license, vehicle liability insurance, and a minimum of two years of safe driving experience
