What are the responsibilities and job description for the Hospice RN Case Manager position at Continuing Care Management?
Seeking a dynamic, talented, caring and compassionate nurse to become part of a TEAM that promotes our patients independence and comfort within their home at end of life. Participate in patient and family teaching to provide safety, security and support.
- Shifts are 8am - 430pm
- Full training will be giving to the right candidate.
