At Copa Health, we are seeking a compassionate, skilled, and dedicated Children’s High Needs Case Manager to join our Integrated Care Team. In this vital role, you will support children and families experiencing complex emotional and behavioral health challenges by coordinating care, connecting services, and helping youth move toward greater independence, well-being, and—when applicable—family reunification.
What You’ll Do:
- Partner with children, families, and their support systems to implement and manage Integrated Service Plans (ISPs) tailored to individual recovery and wellness goals.
- Serve as a liaison between children, families, healthcare providers, behavioral health professionals, schools, and other community partners.
- Provide weekly contact and regular home/community visits to assigned clients.
- Facilitate and coordinate Child & Family Team (CFT) meetings and ensure proper documentation and timely service delivery.
- Collaborate with key stakeholders such as DCS, DDD, probation/parole, and educational systems.
- Continuously monitor treatment effectiveness and adapt services to meet evolving client needs.
- Maintain thorough, accurate, and timely clinical documentation according to Copa Health standards.
- Respond to urgent or emergent psychiatric or medical needs through proper coordination and intervention.
- Attend ongoing team meetings and trainings including CFT, CALOCUS, and other required certifications.
Minimum Education & Experience:
- Bachelor’s degree in behavioral health;
- OR Bachelor’s degree in a related field plus one (1) year of direct behavioral health experience;
- OR Associate degree in behavioral health plus two (2) years of direct behavioral health experience;
- OR High school diploma/GED plus four (4) years of direct behavioral health experience.
- Prior experience working with children and adolescents is highly preferred.
- Must have or be eligible to obtain an AZ Fingerprint Clearance Card within 8 weeks of hire.
- Must possess a valid Arizona Driver’s License, proof of insurance, and meet Copa Health driving standards.
- TB Test clearance is required at the time of hire.
- Bilingual (English/Spanish) is a plus.
