Description
- Begins, completes, and documents on the initial treatment plan, master treatment plan, integrated diagnostic summary, and treatment plan reviews/updates.
- Completes screenings and assessments of new clients.
- Clinician is responsible for completing the following:
- Orientation Note
- Initial Treatment Plan
- Psychosocial Assessment & Psychosocial Update
- Educational Learning Assessment
- Master Treatment Plan, Progress Notes, and Treatment Plan Review
- Discharge or Transfer Summary and a Discharge Note
- Develops individualized treatment plan with each new client.
- Performs individual and group therapy with client/family.
- Attends weekly staffing meetings per schedule.
- Maintains after hours availability to assist clients in crisis, as needed.
- Shares information with team as obtained from clients family, or others as indicated throughout the program.
- Documents in the medical record on an ongoing basis.
- Chart audits based on CVRC policy and procedures.
- Provides education to client/family on community resources.
- Assists client/family with appropriate discharge plans.
- Works with administration to enhance the overall treatment environment.
- Performs other duties as assigned.
