Day to Day Life at Custom
- Provide care for our community of recovering patients, ushering them back to a life of independence.
- Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
- Educate our patients and their families on next steps in continuing care and improvement of day to day life including: nutrition, healthy living, and treatments.
- Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of enhancing patient health.

- Valid registered nursing license (required)
- 1 years of registered nursing experience
- Associate's Degree in Nursing required
