Day to Day Life at Custom
- Provide care for our community of recovering patients, ushering them back to a life of independence.
- Create and follow personalized plan of care in collaboration with RN and the interdisciplinary team.
- Educate our patients and their families on next steps in continuing care and improvement of day to day life including: therapy, excercise, and treatments.

Who you are
- Valid Physical Therapy license (required)
