Employment Type: Full time Shift: Rotating Shift Description: Saint Francis Hospital, a member of Trinity Health Mid Atlantic, is looking for a full-time Pharmacist to join our Pharmacy team in Wilmington, DE! Schedule: Full Time, 1 FTE, 40hrs/week, with day and evening requirements. -Pharmacists work 8 hour shifts on weekdays, 12 hour shifts on weekends. -Pharmacists work one weekend per month. -Holidays rotate among pharmacists. Job Description: The Pharmacist is responsible for providing pharmacy services to patients, physicians, and co-workers through accepted practices, policies, and legal requirements of federal and state regulations. The Pharmacist will provide clinical support services to support a full range of pharmacy operations. Central pharmacy activities includes reviewing patient profiles at the time of dispensing medications, ensuring prescription directions are appropriate, and other duties related to pharmacist services. As a decentralized Pharmacist, you will work collaboratively with physicians and nurses to provide care to the patient. Daily activities include drug therapy monitoring, provision of drug information and drug distribution activities. The Clinical Staff pharmacist will excel in all aspects of the staff pharmacist role and with work with the pharmacy leadership team to promote safety and high quality care through patient facing activities (counseling, medication histories, rounds, etc), protocol development, and continuous quality improvement projects. The successful candidate will support staffing and distribution while spending time on protocol development, patient teaching and interaction, and rounding as part of the multidisciplinary team. Requirements: PharmD from an accredited institution DE licensure as a Registered Pharmacist Candidates must have a pharmacy practice residency and/or experience as a Pharmacist in a hospital setting, Experience with Epic, Pyxis, sterile compounding.
