Position Title: Case Manager
Reports to: Manager of Case Management
Job Classification: Full-time; Non-exempt
Salary Range: $21.88 - $27.31 per hour

The Opportunity:
The Case Manager position is an integral member of the Mission’s team and helps organize patient care. This position will work with healthcare providers to ensure that our member(s) receive the services they need in providing assessments and assignments to provide facility and program information.
Responsibilities will include but not be limited to:
- Demonstrate respect, cultural competence, and knowledge of trauma-informed care in interactions with participants, staff, and other community providers.
- Uphold all residential and DRM program policies and written agreements.
- Develop a case plan with individualized goals based on individual needs, and facility/program requirements. Identify action steps necessary to achieve goals and monitor progress through scheduled follow up meetings.
- Perform comprehensive assessment of participant needs.
- Recommend the necessary level of intervention.
- Develop and regularly modify treatment service plan as appropriate.
- Encourage and hold participants accountable to program guidelines.
- Monitor and assess participant progress towards identified goals.
- Hold case planning meetings with clients, including scheduled and walk-in appointments.
- Monitor Case Planning appointments, schedule new clients, and assess continued availability of participant needs and the current bed list to make sure a maximum number of appointments are offered.
- Work collaboratively to identify and assess escalating crisis situations and respond with de-escalate training technics.
- Maintain professional best practices in areas of document storage, confidentiality and professional ethics when dealing with clients and client information.
- Oversee Participant case files to ensure progress.
- Regularly update all case planning records, services, and case notes in SCRIBE including uploading vital documents and ROIs.
- Document all meetings, referrals, and services in SCRIBE and utilize the appropriate templates.
- Attend staff meetings and be prepared for case manager and team meetings to address complex cases and needs.
- Provide SCRIBE reports, spreadsheets and statistics as requested.
- Facilitate Community Provider visits with clients including case note documentation in Scribe (DHS, MAT, Child Support, VA, SSI/SSDI Attorney).
- Collaborate, communicate, and advocate with service providers in the Government, Medical, Mental Health, Housing, Benefits Acquisition, Employment, Legal, and Veterans, etc. fields to aid clients toward a successful transition into long-term programs or sustainable housing.
- Facilitate Community Provider visits with clients as assigned, to include, signing up clients to meet with Community Providers.
- Collaborate with other DRM personnel regarding participant evaluation, goals and continuity of care.
- Engage in ongoing training as assigned by your direct Supervisor and leadership team.
- Participate in care/integration/oversight of interns and volunteers as requested.
- Seek to develop a continuing knowledge of the field, what services are provided in the community and establishing communication with those entities.
- Other duties as assigned by direct supervisor.
Requirements
Minimal Qualifications:
- Must possess a BA/BS degree, preferably in human services, social work or related field. Four (4) years’ experience in a similar position may replace BA/BS degree.
- CAC certification preferred.
- Abide by the Employee Handbook.
- Affirm the Statement of Faith.
- At least one year’s experience in case management, ideally within an urban setting with low-income populations, preferred.
- Excellent verbal and written communication skills, and responsiveness to communication.
- Ability to resolve conflict Biblically, professionally, and respectfully while maintaining composure under pressure.
- Strong Christian ethic with ability to work with people of diverse cultural, educational, and religious backgrounds.
- Ability to read, remember and follow precise policies and procedures.
- Strong working knowledge of Windows software applications and ability to learn SCRIBE and Clarity systems.
- Willingness to possibly work either Sat/Sun and travel between locations.
- Ability to pass background checks to work with individuals on probation or parole.
- A heart and commitment to serve the needy and seeing lives changed in the name of Christ.
Working Conditions:
- Occasional lifting up to 35 lbs., bending, and climbing stairs.
- Work with individuals who are experiencing homelessness, addiction, mental illness, and a myriad of other issues related to homeless populations.
- Possible exposure to communicable diseases.
- A limited amount of work-related travel may be required occasionally.
