Job Duties

This position supports the facility’s mission by serving as a member of the Personal Support Team and Support Coordinator/QIDP for 5 and up to 10 people. Responsibilities include the coordination of person-centered plans, active treatment, and activities for these individuals, supervision of staff members assigned to three shifts (scheduling, performance evaluation, routine training), and administrative responsibility for the maintenance and upkeep of the home. This position also serves as the direct care staff’s immediate supervisor in the home related to training needs, timekeeper, supervision, observations and evaluation. This position will provide 24 hour coverage for staffing through advanced scheduling in an online template and adjusted as changes occur. Monitoring of overtime, punctuality and use of leave by staff is another responsibility of this position. Ability to clearly communicate on behalf of each individual in the home using Person Centeredness through verbal and written communication. Leadership qualities must be exhibited through interdisciplinary interactions. Serves as Team Lead in all meetings for individuals whom reside in their home. Ability to monitor individual’s status for all disciplines and provides input and feedback. Demonstrated ability to understand CMS/ICFMR and Facility policy and procedures; and regulations for compliance.

This position is considered essential personnel and would be required to work in the event of an emergency or facility need. All SEVTC positions are required to work different shifts and/or overtime as requested.

Minimum Qualifications

A Bachelor’s Degree in one of the behavioral, social science or human service fields such as Special Education, Recreation Therapy, Psychology, Nursing or Social Work required. Must meet the requirements of a Qualified Intellectually Disabled Professional (QIDP). Two or more years’ experience working with people who are intellectually disabled and who may have additional developmental and behavioral disabilities required. Experience with monitoring support delivery for persons with disabilities required.

Additional Considerations

  • Master’s degree in relevant Human Services field.
  • QIDP experience.
