Hiring Bonus: $500
Located on a beautiful 10-acre campus in the heart of Scottsdale, Devereux Arizonas 24-hour Residential Treatment Center (RTC) has the capacity to serve 48 children. The RTC is a safe, supportive environment that provides various services to youth, from the ages of 5 to 17, who are experiencing emotional, behavioral, or substance abuse challenges. The Campus features dormitories, a therapeutic K-12 day school, food services, medical/nursing, and therapy services.
Reporting to the Clinical Program Manager, the Care Coordinator works cooperatively with clinical, operations, medical, and administrative staff on the overall coordination of individual care at the RTC. The Care Coordinator participates in family engagement/outreach, admission/intake processes, discharge planning, treatment planning, and documentation of all care coordination activities accordingly.
Pay Range: $19 - $20.90 / hr
Schedule: Monday - Friday 9:00am - 5:00pm

Qualifications
Must be 21 years of age or older
HS Diploma or GED required; Bachelors degree in Human Services, Psychology, Social Work or relevant field preferred
Four (4) years experience in mental health/social services field with high school diploma/GED OR one (1) year of related experience with Bachelors degree
Must demonstrate excellentverbal and written communication skills
Ability to handle multiple tasks and meet deadlines with minimum supervision; self-motivated
General knowledge of clinical best practices for working with adolescent populations and theirfamilies/resources
General knowledge of mental health, funding and related services within Arizona and other states served.
Generalknowledge of standard office computer applications.
OTHER:
Must be available to attend 2 weeks of required orientation training (paid, Mon-Fri 8:30am-5pm) plus additional 2-3 weeks on the job training
