Summary of Duties and Responsibilities:
Under the general direction of the Home and Community Based Services (HCBS) Director and/or their designee, the Case Assistant provides support and assistance to the Assessment, Care Management, and program staff to ensure eligibility with HCBS programs.
Essential Function(s):
- Monitor, track, document, and follow up with appropriate parties regarding eligibility for PASSPORT, Assisted Living Waiver, MyCare Waiver, Ohio Home Care Waiver, and Specialized Recovery Services Program.
- Works to secure waiver service providers for members enrolled onto PASSPORT, MyCare Waiver, Ohio Home Care Waiver, and Family Caregiver Support Program.
Qualifications
Minimum Job Qualification:
Applicant must have a high school diploma. An Associate Degree or equivalent experience is preferred. Excellent communication skills, good typing and computer skills are required.
Knowledge of community resources with one year experience in the Human Services field is desirable. Applicant must have a valid driver’s license with ability to travel.
Salary : $36,635 - $43,962
