We are looking for a new team member to join us as a Case Manager for our Healthy Families Program. As a case manager, you will be responsible for building and sustaining a relationship with a caseload of diverse families who are first time parents of newborns through the child’s earliest years. The case manager will be conducting weekly home visits primarily within the family’s home, providing services to strengthen the parent child relationship, promoting healthy childhood growth and development, and enhancing family wellbeing. Activities may also include but are not limited to administering tools such as developmental, depression and substance use screenings, and identifying and referring families for other supportive services including mental health services and community resources. This position requires travel throughout DuPage County. Dual language candidates are encouraged to apply.

- Working with and providing services to young children and their families.
- Outreaching families and building and establishing trusting, long term (3-5 year) relationships with families
- Manages a client caseload consistent with department productivity standards.
- Meets with clients in their homes and in the community throughout DuPage County. Also maintain communication with clients via phone, text, and video visits.
- Completes assessments and screenings including parent interview about the family’s story and screenings for developmental functioning, depression and substance use screenings etc. Provide referrals to appropriate resources
- Participates in weekly reflective individual supervision and monthly reflective group consultation.
- Completes and maintains required documentation within established time frames.
- Participates in emergency response activities as assigned.
- Maintains required training, licensure, and/or certification.
- Maintains confidentiality of privileged information and adheres to patient privacy laws.
- Demonstrates sensitivity and understanding of other ethnic groups and cultures.
- Adheres to department guidelines for attendance and punctuality.
- May perform outreach activities.
Bachelor’s Degree in Social Work, Psychology, or a related field in human services. One year of experience in case management, home visiting or an equivalent combination of training and experience and a Valid Driver's License with the ability to drive throughout DuPage County.
Experience and/or willingness to grow in capacity to work with babies and their families.
Knowledge of infant and child development.
Salary : $36,770 - $61,280
