Description
Primary Duties and Responsibilities:
1. Responsible for providing behavioral health counseling to children, adolescents, adults and families in order to improve psychosocial functioning;
2. Provide comprehensive assessments and diagnosis of behavioral health patients;
3. Provide effective treatment planning and assist patients in successfully achieving goals;
4. Evaluate behavioral health crises and apply appropriate interventions;
5. Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease; and,
6. Participate in staff meetings and the quality improvement program.
Requirements
1. Must hold a Master’s degree in social work, counseling psychology or a closely related field
2. Must have 2 years of experience in any clinical occupation providing mental health counseling and interventions to clients/patients in a mental health setting under supervision of licensed mental health practitioner (LCSW/LCPC/PsyD), and
3. Must hold a license to practice as a LCPC or LCSW in Illinois.
