The ideal candidate will have previous experience in case management supporting severe mental health clients, experience working with substance abuse clients, the ability to effectively interact with a variety of individuals and managing caseloads

PRIMARY FUNCTION:
Responsible for providing outreach, engagement, assessment, and case management services to street homeless individuals age eighteen and older in Montgomery County, MD.

ESSENTIAL DUTIES AND RESPONSIBILITIES: *
- Maintains flexible hours to regularly canvass and engage in outreach supports to both potential clients, active clients and community businesses. This includes regular evening and early morning availability along with support of the County PIT or other count efforts and surveys.
- Provides or arranges transportation, as needed, to/from appointments or attending with client when necessary. This does require the use of a personal vehicle capable of daily use and accommodating a client during business hours.
- Maintains records of all client interactions, referrals, written needs assessments, dispositions, and all client related paperwork per reporting guidelines. This includes internal tracking and county electronic records. Timely documenting and tracking all client interactions, related supports, and formulating observations into written needs assessments.
- Effectively utilizes County electronic health record database to enter clinical service plans, contact notes, and intake information. This includes regularly updating client data and profile to match county reporting requirements.
- Advocates on behalf of clients for needs, services, and supports while serving as link to and provide direct assistance with accessing resources, such as MANNA food donations, shelter, housing, financial entitlements, donation sites, DHHS, food stamps, Social Security, medical care, and all other County resources available to the client.
- Coordinates client mental health support and ongoing care with therapists, psychiatrists, and hospitals as needed.
- Submits reports and complies with Program Director requests for data reporting as needed.
- Develops case plans, monitors progress, and reassesses client needs and stage of change.
- Completes and engages in trainings as needed to support client needs and effectively perform duties.

Minimum Qualifications*:

Education:
Bachelor’s Degree in a social services discipline.
Experience:
At least one year of experience in providing services to low-income, homeless, psychiatric, substance involved, or co-occurring adults.
Skills:
Excellent oral and written communication skills. Must be able to organize and prioritize. Experience with Microsoft Office platforms, Electronic Health systems and accurate data entry skills. Bilingual Spanish preferred.

Pay: $50,000.00 - $55,000.00 per year
