JOB SUMMARY:
Face to Face Health and Counseling is a non-profit organization dedicated to meeting the needs of youth ages 11–24. We offer health care, counseling, and programs for homeless youth.
The primary role of the Clinic Case Manager is to provide case management, care coordination, and health insurance enrollment to youth at our Arcade clinic on the Eastside of Saint Paul. The Clinic Case Manager is responsible for health insurance enrollment, medical care coordination, intensive case management for prenatal program participants, and case management for youth requiring additional support or services as referred by the medical, mental health, health education, or SafeZone teams.
RESPONSIBILITIES:
Clinic Case Management
- Interview and evaluate youth needs including intake assessments.
- Assist youth in developing goal plans to meet their needs.
- Coordinate care within Face to Face and in the community to assure continuity and quality of care.
- Accompany youth to appointments or assessments as needed.
- Provide prenatal and parenting education one-on-one, in group settings, and at events.
- Work closely with clinic staff, including participation in work groups, case consultation, and other efforts to coordinate prenatal care.
- Arrange transportation for clients to the clinic and other appointments as necessary.
- Provide ongoing support, resources, advocacy, and crisis management.
- Conduct prenatal and post-partum home visits with OB Program clients.
- Document services provided accurately and thoroughly in Apricot case management database.
- Develop and maintain ongoing working relationships with staff from community organization and agencies serving the same population.
Medical Care Coordination
- Provide assistance to medical and mental health clients in coordinating medical care beyond what they can receive from Face to Face. This includes identifying and contacting appropriate specialists, scheduling appointments, arranging transportation, requesting medical records, interpreting information from providers, etc.
- Coordinate medical services with clinic staff and the OB/GYN providers.
- Follow up with clients who were referred to external resources to ensure they received the assistance they need.
- Develop and maintain a resource list of youth-friendly medical providers/specialists to refer Face to Face clients. Maintain ongoing working relationships with these providers to ensure smooth, warm referrals.
- Accompany clients to appointments, as appropriate.
Health Insurance Enrollment
- Conduct one-to-one MNsure education, enrollment, and follow-up to uninsured youth receiving Face to Face services and uninsured community members seeking insurance.
- Assist with health plan enrollment and reporting changes.
- Maintain up-to-date knowledge of MNsure policies and Affordable Care Act (ACA) rules.
- Partner with other metro youth serving agencies to provide MNsure enrollment assistance.
Other Responsibilities
- Work with the case management team to achieve the goals and objectives of specific grants, including data tracking necessary for these grants.
- Model excellent internal and external customer service.
- Maintain patient confidentiality as required by agency’s policies and HIPAA.
- Model behavior that is consistent with, and supportive of, healthy adolescent development.
- Provide all services guided by the evidence-based principles for helping youth overcome homelessness.
- Step in and help to "get the job done" when short-staffed.
- Perform other duties as assigned.
QUALIFICATIONS:
- Bachelors or equivalent degree in social work or human services. Relevant work or lived experience considered in the place of a degree.
- Minimum of one-year experience in case management preferred.
- Demonstrated ability to work and communicate effectively with people of diverse ethnic, economic, racial and cultural backgrounds.
- The ability to work independently, exercising good judgment in involving others when necessary.
- Positive attitude and ability to work with a team.
- Current MNsure Navigator certification; or the ability to be certified and pass a background check.
- Good crisis management skills and the ability to navigate stressful situations.
- Strong computer skills with a high level of attention to detail.
- A valid driver’s license, current automobile insurance, and a good driving record required.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
- Ability to perform the duties outlined in the job description.
- Availability during all hours of operation: Monday, Tuesday, Wednesday and Thursday from 10:00 a.m. – 6:00 p.m.; Friday 10:00 a.m. – 4:00 p.m.
- Must be able to occasionally accommodate evening and weekend hours.
- Hours may change if clinic hours change.
- Face to Face is subject to the government mandate requiring employee COVID vaccination. All employees will be required to get vaccinated.

PAY & BENEFITS:
The pay rate for this position is between $22 and $22.91 an hour, based on qualifications. Face to Face offers benefits which include health, dental, short-term disability, long-term disability, life insurance, 403b account as well as generous PTO and paid holidays.
