The M Health Fairview Tamarack Clinic is a growing and collaborative clinic. Our clinic hours are 7AM to 6PM Monday - Thursday and 7AM - 5PM on Fridays, so no evenings, weekends, or holidays! This is a 1.0 FTE (80 hours per two week pay period) opening.

The M Health Fairview Tamarack Clinic has an immediate opening for a Visit Facilitator to join our Primary Care team.  Our clinic utilizes a team-based approach to support our Internal Medicine and Family Medicine providers in delivering excellent care to our patients and community. Visit Facilitators are a valuable member of our care team and collaborate with providers and support staff in meeting patient care needs during and between appointments in the clinic. Visit Facilitators on our team gain experience beyond patient care by working on collaborative projects with other clinics in our service line, leading process improvement initiatives in our clinic, serving as subject matter experts in the clinic, and receive ongoing competency and skills training by our clinical leads. This position is responsible for supporting in person clinic visits & helping guide patients through the care experience. Visit facilitators capture pertinent medical information from the patient & details the information into the medical record. This role also helps ensure the exam rooms are stocked, equipment & supplies are available, assists with tasks that support the effectiveness & efficiency of the team. The Visit Facilitator may also assist in scheduling & coordinating appointments, preparing schedules & other duties as assigned.

• Prepare patients for their appointments including: communicating prior to their scheduled appointment to ensure medical records are collected, received & available for visit
• Assist in rooming process by collecting & detailing pertinent information: reason for visit, medication review, vitals collection with automated vitals machine, & health questionnaires
• Supports the patient connection to the provider for the visit & acting as a resource while ensuring a positive patient/provider experience
• Completes visit wrap-up/after-visit summary is provided & patients are aware of recommendations for follow up. This may include assisting with scheduling & coordinating follow-up appointments.
• Communicates efficiently with patients, providers & care team, demonstrating comfort in person & with virtual technologies

Required

• At least 1-year of customer service experience
• Proficient with technology (computer hardware and programs, smart devices such as phones or tablets)

Preferred

• Current nursing, medical, social work, psychology, or pharmacy student
• Successful completion of: NA, EMT, or HUC program
• Experience with Medical terminology, Epic Health Records, Telephonic customer experience and has previously worked in a health-care setting
