What are the responsibilities and job description for the IDD Team Supervisor position at Family Care Services?
We are looking for an individual to supervise a team who provide case management duties to adults with intellectual and developmental disabilities living in private residences. No direct care is provided by this position. - Supervises and provides training to a team of subordinate Life Sharing Specialists.
- Reviewing reports.
- Be available to subordinates and respond by phone/email.
- Fill the role of a Life Sharing Specialist as needed.
- Insure compliance for all team members with applicable regulations, program/service descriptions, guidelines, policies, procedures, practices, and directives.
- Visits all caregiver homes.
- Completes monthly team meetings.
- Maintains compliance standards by routine chart audits.
- Completes all pre-placement requirements for new client and caregiver referrals.
- Insures completion of all required subordinate and caregiver training.
- Provides training and maintains all required certifications.
- Completes incident management duties, including certified investigations.
- Verifies service authorizations and utilization.
- Provides program improvement strategies and assists with implementation.
- Provides initiatives to insure success of the program capacity by recruiting, advertising, and public speaking.
- Be a liaison with natural families, caregivers, day programs, and county agencies.
- Part-time on call duties for emergencies during weekends and holidays.
- Proficient in Microsoft Office
- Detail and deadline orientated
- Excellent written and verbal communication skills.
- Organized
- Good knowledge of Mental Health and Positive Behavior Approaches.
- Good knowledge of all ODP requirements and best practices.
- Good knowledge of Everyday Lives Principles and LifeCourse Framework.
- Qualified to administer The Health Risk Screening Tool (HRST) and has a good understanding of the clinical issues associated with the tool.
- Good clinical skills.
- Good interpersonal skills.
- Good public speaking skills.
- RN, LPN, and/or bachelor’s degree in human services and 2 years work experience working directly with persons with an intellectual disability.
- Master’s degree in a human services field and 1-year work experience working directly with persons with an intellectual disability.
- Certified behavior specialist or have the qualifications to become a behavior specialist.
- At least 2 years of supervisory experience.
