What are the responsibilities and job description for the Care Coordinator - Substance Use Disorders position at Family Health Center?
The Care Coordinator–Substance Use Disorders provides outreach and service coordination for referred patients diagnosed with substance use disorders. As part of a recovery-oriented system of care, the Care Coordinator–Substance Use Disorders works in collaboration and partnership with identified patients, family/caregiver(s), providers and community resources in a team approach to promote recovery of the patient.
ESSENTIAL JOB FUNCTIONS
1. Regular attendance is required in order to carry out the essential functions of the position.
2. Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge, and abilities to perform, within scope, role specific functions.
ADDITIONAL DUTIES
1. Other duties as assigned.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: High school diploma or equivalent.
EXPERIENCE
Minimum Required: Personal and/or professional knowledge of substance abuse and integrated service delivery systems. Experience with communication, facilitation, organizational skills and the ability to multi-task. Must be PC literate, with a working knowledge of word processing.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: State of Wisconsin driver’s license with an acceptable driving record. Current Basic Life Support (BLS) certification or to be obtained within three months of hire.
