Title: Family Advocate
Reports to: Program Director
Hours: Monday-Friday 10am-6pm. 2 weekday evenings per month. 4-5 events a year which could include nights/weekends.
Pay: Hourly Rate, 40 hours a week $23-$28 an hour
Overview
Family Promise Rochester is committed to facilitating the empowerment of homeless and low-income families within the Rochester, Minnesota area, fostering sustainable independence through a community-driven approach.
Responsibilities
Case Management (.6FTE):
- Screen prospective program participants for emergency shelter and diversion and prevention.
- Perform guest intake process to include orientation to the program, needs assessment and development of an individualized case plan.
- Perform guest exit process to include exit interview and moving plan.
- Execute all in-house programming initiatives for the Family Promise Program, ensuring seamless implementation and alignment with program objectives and participant needs.
- Assist Program Director in planning and facilitating Life Skills classes to current and graduate families.
Community Engagement (.1FTE):
- Represent the organization with the local Continuum of Care and other community networking events related to homelessness.
Program Management (.3 FTE):
- Implement and advance the goals of core Family Promise Rochester Programs, ensuring their effective execution and alignment with organizational objectives.
- Execute updates to program policies and procedures as required, ensuring alignment with evolving organizational needs and industry standards.
- Manage daily operations to support program objectives.
- The family advocate is responsible for maintaining up-to-date and accurate client data, diligently recording all interactions, progress, and relevant information. This entails meticulous record-keeping and note-taking to ensure comprehensive documentation of each client's journey and facilitate informed decision-making and effective support provision.
Additional Responsibilities:
- The schedule is anticipated to encompass at least one evening shift per week and one weekend day per month, or a comparable combination thereof. Flexibility in scheduling is essential for this role, as it may necessitate availability during weekends and evenings to accommodate meetings with families and fulfill job responsibilities effectively.
- A valid driver's license is required, along with a readiness to utilize the Family Promise Rochester vehicle for guest transportation, or alternatively, one's own vehicle if needed (with mileage reimbursed).
- The role may involve occasional lifting of materials and donations, as well as assisting with the relocation of families into their own apartments.
- Provide support at fundraisers and special events as required, contributing to the success of these initiatives through various tasks and responsibilities.
- The Family Advocate is required to compile and write a detailed monthly report for the Program Director, which will be presented at the board meeting
Education, Experience and Skills:
- The ideal candidate will demonstrate enthusiasm, sound judgment, initiative, and a commitment to assisting families in achieving positive outcomes. A proactive and adaptable attitude, with a problem-solving mindset, is essential.
- A four-year degree, in fields such as BSW, psychology, or a related discipline, is preferred.
- Candidates should possess a minimum of three years of direct service experience in areas such as employment placement, housing/shelter, or family services.
- Proficiency in case management skills including trauma-informed care, rapid re-housing, housing-first, and a strength-based approach is expected.
- Strong attention to detail, the ability to manage projects in a fast-paced environment, prioritize tasks, and work autonomously while exercising good judgment are crucial.
- A positive attitude is paramount, serving as a role model for the families we assist.
- Proficiency in computer programs and a proven track record of effectively tracking outcomes and documenting case notes are required.
- Comfort working with diverse constituents, including guests, volunteers from faith communities, corporate partners, and foundation partners, is essential.
