Our Adult Day Health Center in Fall River, MA has an exciting opportunity for a LPN; part time hours (21 hours per week) are available.

The ideal candidate will assist the RN as liaison between participant's primary care physician and will monitor participant's health status and care plan. Some of the responsibilities are to complete medical assessment forms, provide case management services as designated by RN; review and supervise participant's personal care services; attend monthly supervisory meetings with RN; writing monthly nursing progress notes in each participant's record; and assist the Program Manager with the implementation of policies and procedures.
Our ideal candidate will have current licensure in the Commonwealth of Massachusetts Board of Registration in good standing, CPR certification and two years experience working with aging and disabled population required; previous experience working with mental health, dementia, veterans', dual diagnosis preferred.
The hours are 8:00 a.m. - 12:00 p.m. Monday through Friday. 21 hours per week.
