What are the responsibilities and job description for the Registered Nurse - Clinical position at Forest County Potawatomi Community?
Here is what you’ll be doing:
- Provides direct, professional nursing care to patients utilizing the nursing process of assessment, planning, implementation, and evaluation.
- Delivers patient-centered care to patients and families by providing care coordination and care management services.
- Performs in-house and phone triage to assess patient acuity and their needs for appropriate and timely access to care.
- Educates individuals and families on recommended services and exams that are due across their lifespan.
- Participates in quality improvement projects and initiatives within the medical department.
- Follows the instituted health division policies and procedures applicable to job position.
- Ensures strict confidentiality of health records.
- Provides care in a culturally sensitive manner to the population served.
- Performs other duties which are delegated and required.
What you’ll need to be successful:
- Bachelor of Nursing (BSN) required; Associate degree considered with prior case management experience of 3 years in a previous healthcare setting.
- 3 years’ experience in a patient care setting of patients across the lifespan is required.
- Valid Registered Nurse license for the state of Wisconsin and basic life support (BLS) is required.
- Ability to provide care in a practical manner according to training, experience and approved competencies.
- Knowledge of applicable federal, state, county and local laws, regulations, and requirements.
- Valid Wisconsin driver’s license required; must maintain a valid Wisconsin driver’s license as a condition of employment.
- Must successfully pass all applicable background checks and drug screens.
- FCP complies with all CDC vaccine and testing recommendations for healthcare professionals. Proof of immunizations is required and immunizations/testing provided upon hire.
Preferred qualifications:
- Ability to provide care in a practical manner according to training, experience and approved competencies.
- Knowledge and experience providing care coordination and care management services.
- Knowledge of health education theory and practice
- Knowledge of chronic conditions and preventative care for patients across the lifespan
- Experience working with electronic medical record systems.
