What are the responsibilities and job description for the Public Health Nurse II position at Forsyth County (NC)?
Forsyth County Department of Public Health is searching for Public Health Nurse II to join the team. This Public Health Nurse II position will serve as the High-Risk Perinatal Coordinator to plan for the coordination of care provided to high-risk maternity patients in a multi-county area and serve as the liaison to Downtown Health Plaza (DHP).
The ideal candidate will possess the following knowledge, skills, and abilities:
Considerable knowledge of and skill in the application of nursing theory, practices, principles, and techniques employed in the field of public health and related programs;
Working knowledge of and the ability to apply the principles and practices of public health;
Working knowledge of current social and economic problems relating to public health;
Knowledge of available resources and organizations in the community;
Ability to record accurately the services rendered and to interpret and explain records, reports, and medical instructions;
Ability to establish and maintain effective working relationships with other employees;
Ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions and;
Ability to secure the cooperation of clients, to elicit needed information, and to maintain effective working relationships. Graduation from a four year college or university with a B.S. Degree in Nursing which includes a public health nursing rotation and one year of professional nursing experience in specialty area; or
Graduation from an accredited school of professional nursing and two years of professional nursing experience in specialty area assigned.
Must be licensed to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing.
A higher education level may be considered as a substitution for all or part of the experience requirement.
Individuals hired without a rotation through a public health department as part of educational requirement will be required to complete Introduction to the Public Health Nursing course within one year of hire.
A valid Drivers License is required.Essential duties and responsibilities include but are not limited to the following.
Attends quarterly Continuity Care Conference on women with a history of a recent poor birth outcome based on obstetrical history.
Convenes quarterly audit meetings for the DHP High Risk Maternity Clinic record review and compiles results to be available to the Women's Health Section.
Reviews prenatal records to identify clients and refer to High Risk Maternity Clinic services, social work services, and nutritional services and maintains a central file of DHP OB patient's registration cards with pertinent demographic information and obstetrical information.
Assists in making postpartum/family planning appointments.
Provides written information to OBCM's as requested, regarding delivery, discharge, special instructions and appointments for mother/baby.
Obtains referrals on those patients who have received no prenatal care.
Updating, reviewing and as needed developing policies and procedures needed to meet state requirements each fiscal year. Communication and assisting implementation of those policies and procedures with DHP staff.
