Position Title: Stabilization Case Manager
Program: Friendly House Shelter & Scattered Sites
Location: Worcester
General Summary of Position: The Stabilization Case Manager will assist in ensuring a successful transition into permanent housing and will work closely with families to build independence and continued stability.
Major Responsibilities:
Meet with each family prior to the transition to permanent housing to identify their goals, objectives and particular areas of interest and needs.
Develop a stabilization plan and record necessary information/progress notes in family’s files on monthly basis.
Collaborate with shelter staff, community providers to monitor family plan.
Meet with families at their home once a week or more often, if necessary.
Assist families with daycare needs, transportation, and ongoing case management on an as need basis, to build independence and stability. Collaborate with financial institutions for budgeting needs, assist with arranging financial savings.
Make appropriate referrals to community agencies.
Participate in all site team meetings with involved supportive services and program staff.
Maintain updated case file and documentation
Maintain data entry into HMIS
Complete re assessment tool for each family in case load every 45 days
Provide transportation to appointments, when necessary
Perform other duties as assigned by Director of Shelter Services
Qualifications:
Bachelor’s Degree/ two years human service experience, bi-lingual verbal and written skills, sensitivity to working with low income families, knowledge of housing issues and strong case management skills. Must have transportation
