SHIFTS:
3:00 p.m. – 11:00 p.m. (4 days on 2 off rotation)
OR
11:00 p.m. – 7:00 a.m. (4 days on 2 off rotation)
ESSENTIAL FUNCTIONS: Provides assistance to residents with ADL’S and IADL’S and assists in the maintenance of the facility environment. Participates actively in the provision of care services to the resident.
ADDITIONAL FUNCTIONS:
- Receives and gives report to charge nurse & team members at change of shift.
- Encourages or assists with activities of daily living (ADLs) such as bathing, grooming and with instrumental activities of daily living(IADL) such as shopping, housekeeping, laundry, socialization
- Practices effective communication techniques with residents, visitors, and coworkers and promotes a positive image of the facility
- Arranges, schedules, encourages resident activities, both spontaneous and scheduled
- Adheres to infection prevention and control practices
- Assists with admissions, transfers, and discharges of residents
- Prepares apartments for new admissions
- Transports and escorts residents
- Mentors new employees
- Documents care as necessary
- Participates in development, implementation, and evaluation of residents’ service plan
- Keeps environment, equipment, supplies, and resident furniture clean.
- Reports to supervisor:
- Changes in residents’ status
- Equipment and supply needs
- Repair and maintenance needs
- Safety hazards
- Unusual occurrences or observations
- Problems with completions of assignment
- Directs resident and visitor concerns to supervisor
- Responsible for obtaining required in-services
- Adheres to posted schedule. Submits requests for time-off per facility policy
- Responsible for adhering to all safety guidelines for protection of self, residents, and co-workers
- Works to promote maintenance of residents’ abilities at their highest level.
- Protects residents’ privacy
- Responds to emergency calls from residents
Other related projects, duties and responsibilities may be assigned as deemed necessary.
