Job Summary:
The Chronic Registered Nurse is responsible for delivering and coordinating dialysis care for patients with End Stage Kidney Disease (ESKD). This role encompasses assessment, planning, intervention, and evaluation across all stages of treatment. The RN may delegate responsibilities to other care team members and supports quality improvement and patient education efforts while ensuring safety, regulatory compliance, and excellence in clinical outcomes.
- Assess and document patient status before, during, and after dialysis treatment.
- Implement, monitor, and adjust treatment plans under guidance of nursing leadership or physicians.
- Administer prescribed medications and document justification and effectiveness.
- Educate patients and families on treatment options, including transplant information.
- Collaborate with team members to ensure safe, effective care delivery.
- Supervise and direct Patient Care Technicians and other clinical staff as required.
- Participate in training, orientation, and emergency response measures.
- Ensure accurate lab collection and specimen processing procedures.
- Maintain regulatory documentation and contribute to facility quality assurance activities.
- Minimum 1 year of Nephrology Nursing experience within the past 2 years
- Minimum 3 months of chronic or acute hemodialysis experience
- Minimum 9 months of overall RN experience
- Supervisory experience of clinical staff is preferred but not required
- Mentoring or precepting new RN staff
- Familiarity with interdisciplinary care coordination
- Previous experience with dialysis equipment and EHR systems
