Position Title: Behavioral Health Case Manager II
Pay Rate: $23.50/hr
Department: Department of Health and Social Services
Division: Division of Substance Abuse and Mental Health
Schedule Monday – Friday 37.50 hrs.
The incumbent will work as a part of an interdisciplinary team in the PROMISE program serving individuals with sever and persistent mental illness including those with co-occurring disorders. Incumbents will act as a Care Manager in performing assessments, helping develop individualized recovery plans, determining appropriate level of care, placing individuals in those levels of care and then ensuring that care is delivered according to standards. Other duties will include visiting beneficiaries in the community, charting, and acting as a liaison with a large network of community providers. ** Must have a valid drivers license and be able to operate a vehicle.
Qualifications: Bachelor’s Degree and Valid Drivers License.
