Program Summary

One Day AT A Time (ODAAT) is a partner of the Urban Affairs Coalition (UAC). ODAAT houses men and women who are afflicted by addiction and homelessness. Case management services and prevention for HIV/AIDS is also offered at select locations. Through our holistic array of services ODAAT, consumers can expect the support of ODAAT staff to bring systems together to help achieve positive outcomes. Staff offer support in the following areas: recovery coaching and mentoring, linkage to health and social services, transitional housing, and community outreach. 

Position Summary

ODAAT Testing and Linkage to Care team has secured funding through the Commonwealth of Philadelphia.  A Prevention Grant to provide Status Neutral HIV Testing. The City of Philadelphia Department of HIV Health (DHH) manages contract. ODAAT seeks a staff member interested in Health Education counseling, HIV prevention, and PrEP. Targeted population for ODAAT is Persons who inject drugs and heterosexual Black Women and Latinx Women. 

Primary Responsibilities

ODAAT Health Navigator is responsible for ensuring the successful planning and execution of ODAAT testing goals and objectives guided by The Department of Public Health Community Plan to End the HIV Epidemic. 

Community Based Engagement consists of but is not limited to facilitating partnerships with other Community Behavioral Organizations, hospitals, and local and state departments as necessary. Health navigator will be part of any committee such as the HIV Planning Council or collaborations as requested. With intention to present and orchestrate workshops or attend events to conduct outreach.

Community Based Status Neutral Engagement of Priority Populations consist of but are not limited to providing culturally competent anonymous and confidential HIV antibody testing to Persons who inject Drugs and Black/Latinx Women. Counsel given through a harm reduction model. Navigator will conduct intake, assessment and risk reduction interventions for sexually active clients at significant risk of HIV exposure. Provides PrEP and nPep referral support to clients deemed appropriate for this type of intervention. Will assess for service needs, in particular medical care and medical insurance. 

The Health Navigator is expected to work as part of a team with other Prevention Testers. The Navigator is responsible for working with the team to assure that clients have been linked to appropriate services. This includes ongoing supervision, case conferencing, training and ongoing information sharing among team and referral agencies. Significant to this role is to provide a range of navigation services to link or re-link HIV positive clients to medical care and a range of social services designed to improve retention in care and adherence to HIV treatment services. Provide navigation services to assist in linking HIV-negative clients to prevention, clinical care, and social services. Increase awareness of and access to pre-exposure prophylaxis (PrEP) and non-occupational post-exposure prophylaxis (nPEP). 

Data Reporting in this role will be both qualitative and quantitative. Use of software and Microsoft Suite programs such as excel and word is required. The individual selected for this position will report to the team supervisor and assist with the development and oversee the day-to-day data collection operations and submission to key stakeholders. 

 Qualifications and Skills

  • BSW or related degree preferred 
  • Demonstrated expertise in counseling, public speaking and client advocacy. 
  • Preferred experience in delivering structured interventions 
  • Knowledge of resources in Philadelphia or willingness to learn, including educational, behavioral health and health insurance providers
  • Ability to work with diverse populations in critical and emergency situations. 
  • Sensitive to cultural, economic, gender, and sexual orientation differences. Knowledge of local resources and case management service systems. 
  • Understanding or willingness to learn medical and psychosocial issues related to Minority Women, Persons who inject drugs and some knowledge with youth HIV risk factors, disease and related conditions. 
  • Flexibility needed at work sites, work hours may change with prior notice such as evenings and weekends for events. 
  • Ability to work in a team and to collaborate with external agencies to achieve overall project goals. 
  • Familiarity with client confidentiality

  • The noise level in the work environment is usually moderate. 
  • Work will be significantly in the community. Travel is throughout Philadelphia County. 
  • Approximately 10 hours per week will be spent in the office. 
  • Position may require occasional trips to attend conferences seminars, and meetings. 
  • Certain visits or work-related appointments might be scheduled outside of traditional work hours as necessary. 

 

The candidate for this position may be subject to any of the following screenings as part of the pre-employment process: Motor Vehicle Report (MVR), PA Criminal Background Check, PA Child Abuse Clearance, FBI fingerprint, and drug/alcohol screen.
