The Supportive Housing Coordinator is responsible for working in cooperation with the local Housing Authorities (HAs) to determine program and services eligibility of the residents within the housing community through a consumer centered approach to assess, acquire services, reassess and monitor the services provided to elders residing in a senior housing complex and neighboring community. The Supportive Housing Coordinator is situated at the senior housing complex and is available to provide information, referrals, service coordination, activities planning, and follow-up to all building residents.

  • Work closely with all residents to link them to all available resources in the community including, but not limited to: Home Care, Senior Care Options, PACE, One Care, VA services, and services available through state agencies such as the Department of Mental Health and the MA Rehabilitation Commission.
  • Assist in the coordination of a congregate meals program at the Supportive Housing site;
  • Provide social and recreational activities tailored to the interests of the residents;
  • Remain up to date with knowledge of housing related rules, move in and out procedures, application processes and fair housing laws including participating in current trainings, information sessions and seminars to obtain the most consistent interpretations of state and federal regulations
  • Act as the liaison between the HAs, COAs, Provider Agencies and GSSSI.
  • Support the residents in a community living situation, including negotiating concerns among residents;
  • Coordinate on-site 24/7 emergency coverage of the Supportive Housing site, in collaboration with GSSSI's identified vendor agency;
  • Disseminate information to residents by creating newsletters, calendars, and/or information flyers that includes activities, events, and contact information for the SHC, 24/7 emergency contact, and maintenance;
  • Assess residents' functional, health and income status to determine eligibility and appropriateness for community long term care services or programs.
  • Ensure residents have access to public benefits and other community services including SNAP, local food pantries or community food resource, health care insurance and benefits, transportation services,
  • Work with local providers to address unmet needs such as clothing, furniture, durable medical equipment, etc.;
  • Assist with services difficult to access such as mental health, dental insurance, and audiology assistance;
  • Mediate conflicts among residents or between residents and agencies and negotiating satisfactory resolutions;
  • Assist with mail that is difficult for those with English as a second language to understand;
  • Connect residents to employment, civic, or volunteer opportunities.Complete all application materials, quarterly and annual assessments, and progress notes as required by EOEA regulations.

Minimum Qualifications: BA/BS degree in Human Services, Social Services or related field preferred but NOT required. MA Social Work license is desirable. Must have valid driver's license and/or reliable transportation and good driving history. Requires relevant experience up to and including one year.
