If so, Greenfield Rehabilitation Agency is seeking an exceptional Senior Community Coordinator/Occupational Therapist to join our team! In this role, you’ll serve aging adults in Senior Communities and Skilled Nursing Facilities throughout Sheboygan/Calumet County and its surrounding communities.
This position focuses primarily on patient care while also providing valuable support to our Regional Director in overseeing our talented therapists. As the main point of contact for the Administrators of our partner facilities, you’ll play a key role in promoting our services to both new and existing partners. This is a fantastic opportunity for an experienced Occupational Therapist to leverage and mentor their skills, fostering strong clinical outcomes in the most meaningful setting—within the patients’ homes.

Qualifications

Graduate of an accredited school with a minimum of a bachelor’s degree in Occupational Therapy.

Certification or license as required by the State of Wisconsin or applicable state to practice as an Occupational Therapist.

At least 1 years clinical experience
