Exciting Opportunity: Senior Community Coordinator / Physical Therapist – Sheboygan & Calumet County, WI

Greenfield Rehabilitation Agency is seeking a skilled and motivated Senior Community Coordinator / Physical Therapist to support our growing network of senior living communities and skilled nursing facilities in Sheboygan and Calumet County, Wisconsin.
This unique position blends direct clinical care with team support and relationship building, making it ideal for a Physical Therapist who enjoys mentoring others while continuing to provide high-quality therapy services.
Position Overview:
In this role, you will:

Provide individualized physical therapy services to aging adults

Support and collaborate with therapists across multiple community sites

Act as the primary contact for facility administrators and partners

Promote Greenfield Rehab’s services and values to new and existing partners
-
Work closely with our Regional Director to ensure clinical excellence and operational success
This is a highly rewarding opportunity for a clinician who enjoys both leadership and hands-on patient care in a community-based setting.
Qualifications:

Graduate of an accredited Physical Therapy program (Doctorate or minimum of a bachelor’s degree required)

Current license to practice Physical Therapy in the State of Wisconsin

Minimum of 2 years of clinical experience preferred

Strong leadership, communication, and organizational skills
-
Experience in senior care settings (SNF, ALF, Home Health) is a plus
