RN Case Manager

Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our hospice team. The ideal candidate will play a crucial role in coordinating patient care, ensuring caring and compassionate end of life care, and providing support throughout the process. This position requires a strong background in nursing along with previous hospice experience.

Duties

• Assess patient needs and develop individualized care plans based on clinical evaluations.
• Coordinate with multidisciplinary team: Social Work, Chaplain.
• Manage medical records and documentation to ensure compliance with hospice regualtions.
• Provide education and support to patients and their families regarding treatment options and care processes.
• Monitor patient progress and adjust care plans as necessary to meet evolving hospice care needs.

Skills

• Proven experience in case management within the hospice care environment.
• Strong understanding of hospice care and patient outcomes.
• Proficiency in managing medical records with attention to detail.
• Excellent communication skills for effective collaboration with healthcare professionals, patients, and families.
• Ability to work effectively under pressure within the death and dying environment.
• Compassionate approach to patient care, demonstrating empathy and understanding of diverse patient needs.

Job Type: Full-time

Pay: $75,000.00 - $80,000.00 per year
