What are the responsibilities and job description for the Permanent Supportive Case Manager position at Health Care Center for the Homeless?

PSH Case Manager
Responsibilities:
Qualified candidate will provide comprehensive case management services to homeless and indigent clients determined to have higher acuity including assessment, case planning, evaluation of services, and housing placement.
Practices fidelity to the Housing First Model with elements of Intensive Case Management and Assertive Community Treatment team approaches.
Balances and maintains a caseload of 10-15 clients.
Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, development delays, serious mental illness, traumatic brain injuries, literacy issues, and/or other conditions or situations that have impacted housing stability.

Orange Blossom Family Health is a Federally Qualified Health Center founded in 1993. The clinic provides medical, dental, vision, and behavioral health services as well as onsite pharmacy services for our patients. We serve individuals and families in Orange, Seminole, and Osceola counties.
Minimum Requirements:
Bachelor degree in Human Services, and or certification in Case Management with commensurate experience.
At least two years experience in Human Services field providing supportive case management services to at risk, indigent, and or homeless population.
Experience with VI-SPDAT preferred.
Experience with HMIS preferred.
