What are the responsibilities and job description for the Wraparound Facilitator position at Health Connect America?

Position Duties:
1. Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
2. Develop service plans with the input of the treatment team and the Clinical Supervisor
3. Connect families with needed and available community resources. Follow-up
with clients and agencies as appropriate to document use/success of referral.
4. Complete documentation within established time frames
5. Meet with clients weekly or bi-weekly for up to 12 months.
6. Attend and participate bi-weekly with Treatment Team meetings
7. Serve as a liaison with other professionals and agencies as related to the client
8. Actively recruit new referrals to ensure that your caseload stays full
9. Provide 24/7 on-call support for clients as per program requirements
10. Ensure compliance with all state regulatory bodies
11. Participate in the PQI process and strategic planning process
12. Employee will include sensitivity to the service population's cultural and socioeconomic characteristics in providing treatment services.
13. Employee must Independently transport self in his/her own vehicle to provide services in the in- home/community based service milieu, attend meetings; and, if absolutely necessary, transport clients occasionally. Employee must agree & accept this job duty requires him/her to maintain a current driver’s license, a safe driving record, current vehicle insurance; and, maintain his/her vehicle in safe operating condition at all times.
14. Perform other duties as assigned to support operations and mission of the Company.
Qualifications:
Bachelor’s Degree in human services discipline, including, but not limited to: Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc.) and experience working with children and families in case management type/ community resource position. Must be certified by DMH as a CSS.
