Home Health RN Case Manager— up to $89,440/year

HealthCare Support is seeking an independent Registered Nurse (RN) to join a mission-driven team as a Home Health Case Manager in the St. Paul–Minneapolis Metro Area. This full-time, salaried role offers autonomy in the field and the satisfaction of supporting clients in their homes—backed by a collaborative, non-micromanaged team. Territory

• Twin Cities Metro Area — patient visits are prioritized closest to your home base
  
  

Schedule

• Monday – Friday, 8:00 AM – 4:00 PM
• Flexible based on field schedule with a paid one-hour break
• No on-call or weekends
  
  

Compensation

• $71,884 - $89,440/year (based on experience)
• Salary-exempt position, not pay-per-visit
• IRS mileage reimbursement

Daily Responsibilities

• Conduct home visits to assess clients, develop care plans, and oversee caregiver services
• Collaborate with a supportive team that manages scheduling and paperwork while you focus on patient care
  
  

Qualifications

• Active RN license in Minnesota
• Valid Minnesota driver’s license
• Experience in home health, home care, or other field-based nursing roles
