SUMMARY DESCRIPTION OF WORK
This position works as part of the human trafficking team working in close collaboration several community partners ensuring that survivors can access program services with minimal barriers. The trafficking case manager will work in the crisis stabilization shelter they will provide comprehensive case management services designed to meet needs related to immediate safety, trauma recovery, and on-going support for survivors of human trafficking. This position advocates on survivor’s behalf with all relevant government and non- governmental organizations.
Intensive case management responsibilities begin at this level. Decision making, problem solving, and highly developed interpersonal skills are critical. This position requires high level of social work skills, practice, and knowledge.
Compensation range: between $19.74 and $22.00 per hour (wage is determined by years of experience)
Work Schedule: Tuesday through Saturday, 11:00 a.m. to 7:00 p.m.

MINIMUM QUALIFICATIONS
- Bachelor’s Degree in Social Work or Human Service field, AND two years direct care experience in Human Services preferred; OR equivalent employment experience may be considered
- Valid Drivers License and acceptable driving record
Essential Duties and Responsibilities
- Provides crisis intervention and support to survivors of trafficking.
- Demonstrates advanced crisis intervention skills while working with a diverse clientele.
- Assist with meeting basic needs at a crisis stabilization shelter; including assistance with developing and revising an ongoing safety plan, and connecting to medical, therapeutic, or other provides as identified through the intake and assessment process.
- Assists the survivor in identification of strengths, needs and goals. Develops an individualized stability plan in cooperation with the survivor. Monitor and evaluate program participant progress.
- Provides case management services related to meeting the service needs of program participants and helping them obtain self-sufficiency.
- Accurately and thoroughly complete all required paperwork and data entry per agency policies and funding guidelines and within prescribed timelines.
- Assist in organizing and delivering service provision activities to include skill groups, basic needs activities, and recreation.
- Provide client transportation in personal and agency vehicles.
- Participate in weekly team meetings with HFS program staff.
- Follow program policies and procedures; including agency protocols related to medication administration.
- Creates, maintains and shares as appropriate a dynamic self-care plan.
- Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
- Essential functions of this job is to be performed on company physical work site
- In town travel is required, includes use of personal vehicle.
- Is able to work both independently, with little supervision, and as a part of a team.
- Able to establish regularity and dependability in scheduling all appointment and accuracy in reports and statistics.

Preferred- 2 year(s): Previous related experience in human services (with Bachelor's Degree).
